phases in grain

Hello-
I've been trying out the grain opcode and find it quite flexible- it's
handy to be able to modulate the start point and density with your own
processes, not just rand, although that's the first thing to use.

One thing I'd like to suggest be changed, however, is the last argument.
The spec states that this is a flag- if set the phases are random, of 0
phases start at 0. One application is to have a longer sample like a
struck piece of aluminum for the grain source and then slowly meander over
its surface over time by modulating the start point. If this last phase
argument was starting phase, either from 0 to 1 or 0 to 2pi, then this
would set the start point of the grain in a wavetable, either a short
single-cycle wave or a longer sample. This could then be modulated by a
ramp with random added in.
